Specialist accomodation services

 PCS offers accommodation for NDIS participants who require specialist housing solutions to assist with the delivery of supports that cater for their significant functional impairment and/or very high support needs. PCS, with its highly experienced care workers, is a registered provider of specialist accommodation support including but not limited to :-​

  • support participants to live in their own home, or the family home
  • outreach support to assist people to live more independently in their own home
  • shared supported accommodation for people with high support needs and require rostered care

​PCS stands out from the rest of service providers through its reliance on a person-centered approach where services delivery is tailor made to suit the needs of the participants 

Support co-ordination

PCS provides support co-ordination including assisting the participant get ready for their plan review by helping them :-​​

  • assess whether they achieved their goals and got value for money for their plan to identify solutions to problems experienced in implementing the plan to consider new goals
  • consider new goals and decide on what actions to take to achieve goals in relation to exploring housing options and life transition planning

Community nursing

PCS community nursing provides clinical nursing and/or personal care services to participants in their own home as part of the NDIS scheme. Community nursing services can assist with medication, wound care, hygiene and help with showering or dressing. Community nursing services can help to restore or maintain your health and independence at home and assist you to avoid early admittance to hospital or residential care.​ PCS will provide a registered nurse to assess your clinical need for nursing and/or personal care services. A registered nurse will then determine, in consultation with you, the number, type and frequency of nursing and/or personal care services you need.​ These will be monitored and reviewed at least monthly by a registered nurse to ensure the care you receive is appropriate to your clinical needs

Therapeutic services

 PCS provides a variety of specialist therapeutic services supports through NDIS to assist participants gain skills to improve their independence. The services we offer include :-​

  • language and communication
  • personal care
  • mobility and movement
  • interpersonal interactions
  • changes in the person’s functioning
  • community living to support positive development and well-being

  Professionals include: -  

  • nutritionists
  • occupational therapists
  • speech therapists
  • physiotherapists
  • health (including registered nurses, social workers and allied health professionals)

Educational & employment support

 PCS is there to provide assistance to participants to enable them to :- 

  • attend and participate in school or educational programs
  • enable a participant to transition to and from education while incorporating their personal care needs

PCS can provide support to participants to enable them to :​-

  • find employment
  • maintain their employment
  • support  them during the course of their work
  • providing transport to and from work

Plan management

  PCS will support participants with the implementation and coordination of a support plan. NDIS participants can choose how to manage the funding for the supports in their plan. We will ensure that participants remain in control of when and how their supports are delivered. We will ensure that participants remain in control of when and how their supports are delivered. PCS is there to ensure that you get the best services and supports to suit your needs and are responsive to your changing circumstances. Utilizing our key worker model, PCS will provide a dedicated coordinator to personally work with you to ensure your goals are met
